Can anyone tell me their experiences positive and negative with using Prialt with a pump.

I am a spinal cord injury C4 ? C6. I have very bad nerve pain on my right shoulder and down my right arm as well as pain in both legs from my knees down to my toes.

First discovery in United States of colistin resistance in a human E. coli infection

The Multidrug Resistant Organism Repository and Surveillance Network (MRSN) at the Walter Reed Army Institute of Research (WRAIR) characterized a transferrable gene for colistin resistance in the United States that may herald the emergence of truly pan-drug resistant bacteria.
Colistin is the last agent used to combat bacteria that are resistant to the strongest antibiotics. Colistin has remained the best tool available to treat multidrug resistant bacteria because bacteria were not exchanging genes for its resistance. This latest discovery shows that colistin may be losing its effectiveness in antimicrobial therapy. Now, bacteria may be exchanging resistance genes for colistin.
Alarms sounded in the microbiology community in late 2015 when the first transferrable gene for colistin-resistance was identified in China. Since the report, the global health community has monitored and searched for the occurrence of this gene in the food supply and in humans. This colistin-resistance gene has been reported in Europe and Canada and, as of now, is reported in the U.S.
A clinical sample from a urinary tract infection was collected from a patient in a military treatment facility in Pennsylvania. The sample was sent to the Walter Reed National Military Medical Center (WRNMMC) where colistin susceptibility was tested. The results showed that no safe dosage of colistin would be effective to treat such a bacterial infection. WRNMMC recognized colistin-resistance and sent a sample to WRAIR's MRSN for sequencing, which identified the colistin-resistant gene, mcr-1.
"Colistin is one of the last efficacious antibiotics for the treatment of highly resistant bacteria. The emergence of a transferable gene that confers resistance to this vital antibiotic is extremely disturbing. The discovery of this gene in the U.S. is equally concerning, and continued surveillance to identify reservoirs of this gene within the military healthcare community and beyond is critical to prevent its spread," reported Dr. Patrick McGann, MRSN, WRAIR.
Through intergovernmental communication, it was learned the CDC and USDA are also reporting a swine intestinal infection with a single mcr-1 positive E. coli strain. While there is no evidence that links these recent findings, the evidence of the strain in the U.S. is a public health concern. The gene is transferrable to other bacteria, which could worsen the current global crisis of antimicrobial resistance.
An urgent public health response is underway to contain and prevent potential spread of mcr-1. Active surveillance of multidrug resistant organisms (MDROs), such as mcr-1, allows for earlier and more accurate identification of originating sources. The collection and storage of isolates and samples in the MRSN's growing repository helps researchers identify trends in resistance and prevalence of MDROs and provide best practices for medical providers. The repository also enables them to compare isolates from previous occurrences to better respond to future findings. Recognized as a model program by the White House, the MRSN is a key component of the National Action Plan for Combating Antibiotic Resistant Bacteria (CARB).

With the MRSN's archive, this isolate will be archived for future studies to identify new countermeasures. "Through our surveillance system, we have the unique ability to coordinate source information with susceptibility and sequencing data, and if need be, go back to understand changes in infecting organisms to best treat infection and track emerging multidrug resistant organisms," COL Emil Lesho, Director of the MRSN, WRAIR.
This finding has been published by Antimicrobial Agents and Chemotherapy (AAC) of the American Society for Microbiology (ASM).




C6 SCI Detected with Kidney Hydronephrosis

Hello,

Fellow SCI friend and all Dr who are in the Panel.

I am 37yrs old Male C6 SCI quadriplegic since 12yrs from Mumbai, India.


Recently last 3-4months i was having recurring UTI problem. Anti-biotics were not helping much. UTI will come back in few days after completing anti-biotics course. So i consulted my Urologist Doctor, who told me to do blood, urine culture and KUB sonography tests.
Results were
Creatinine = 3.0 (it has been same last 2-3yrs of blood tests......i never took it seriously)
Urine culture = bacteria
KUB sonography = LOT OF DAMAGE IN KIDNEY SHAPE, SIZE, FUNCTIONS AND HYDRONEPHROSIS


So, then i consulted a very very Senior Urologist Doctor in a big hospital here in Mumbai. He further made me do Urodynamics, Kidney Renogram(radioscopy) tests .......Again the tests results are not good..


As per doctors review, urine reflux (urine going back to kidney when bladder full) and infections has done alot of damage in URETER pipe and Kidneys.
As of now, they have given me medicines for infections, spasticity, urine reflux. I have to take medicines for 2months and after that, if the Doctors are not satisfied with progress, have suggested SURGERY to CHANGE URETER PIPES.


I WANNA KNOW, IF ANY OF MY FELLOW SCI FRIEND HAS HAD SUCH PROBLEM BEFORE AND HOW HE/SHE CURED .
ALSO , IN THE PANEL OF DOCTORS IF A UROLOGIST CAN REVIEW MY REPORTS AND SUGGEST ME WHAT TO DO NEXT ??

RT300 FES bike settings? One leg is stronger than the other

I do not expect my legs to be perfectly symmetrical but when I started exercising on the RT300 the circumference of my left leg was 14.75 inches and my right leg was 15 inches. It has been 3.5 years that I've been exercising on the bike. I happened to take a picture the other day and was surprised by the difference in size. The left leg is 16.5 inches and the right leg is 18 inches.

I have misplaced my manual, when adjusting the RT 300 settings how much of an increase should I start with? Anyone have any suggestions?

A local television news broadcast did a piece this week about abuse of parking placards in California. The startling fact was that 1 in 9 California drivers have a handicapped placard. I wondered at the time if there was something wrong with the statement. Maybe the reporter meant to say that 1 in 9 drivers had access to a handicapped parking placard. I guess either way there are a lot of drivers using handicapped parking spaces in parking lots, garages, and on street parking in our state.

Bladder Issues - Help Needed

Good afternoon everyone ! I hope everyone is having an awesome Friday. Last Wednesday I had a bladder botox procedure done because I kept feeling like I needed to use the restroom (my lower abdominal area would get very tight and sometimes spasm). I would need to use the restroom every 2 hours or sometimes more than that. I cath myself on a normal basis every 4 hours. Also, I was getting decent amounts of urine out when I cathed every 2 hours (sometimes 500-600 cc's) when I usually get that every 4 hours.

Since having the bladder botox procedure, I'm still having the spasms and high output. I'm actually leaking more during the day and a heck of a lot more during the night then I was before the botox. I have an appointment with the urologist doctor next Tuesday. I was wondering if you all have any suggestions on questions or comments that I could ask/pass along to the doctor that might help me out. Right now I'm taking 15mg of Ditropan XL 2 times a day for my overactive bladder.

Help me heal my sore!

Hey guys! So Tuesday night I got in bed and found this lovely sore on my left cheek. I peed my pants on a 2 hour trip home from Boston and that's what I'm assuming caused this. It looks much better than it did, it was all blistered on Tuesday night. It was oozing, but has since stopped. I have been staying in bed for these past few days, which is driving me nuts, I've never had a sore like this in the almost ten years I've been hurt. Any tips for healing this thing ASAP? Is it a stage 2 sore? Can I get up at all or should I continue bed rest? I'm gonna attach a pic to this thread, any help would be AWESOME!Name:  image.jpg

Quantum 6000z batteries

Does anyone know how to replace the batteries in the quantum 6000z powerchair? The manual says to lift the footrests up and out of the way and remove the front panel. If the batteries are dead, how can I lift up the footrests? The chair I have is the same one as in this video.

Fractured my left leg, yet again. Do wheelchair seatbelts actually help?

Third time I've fractured my left leg: once 17 or 18 years ago (mid femur fracture from twisting awkwardly), once 4 years ago (distal femur fracture from a friend falling on me), and then yesterday, a proximal tibia fracture, from falling out of my chair after hitting an uneven path in the sidewalk. To say this is a frustrating inconvenience would be a bit of an understatement. Stuck in a fiberglass splint for now until I can actually see an ortho next week.

Something I'm curious about though: is it better to not have a seatbelt and fall out of your chair but (usually) be able to catch yourself, or, to have a seatbelt and go down with your chair (this might also be less likely)? I've always assumed the former, but after this incident, I'm starting to wonder about that. Falling with the chair at least means you have some sort of rigidity to form?

NEED HELP Installing Frog Legs on invacare prospin x4

Is anyone familiar with Invacare's fork/castor assembly on their prospin x4?

I have a bent fork and I purchased frog legs as a replacement, however none of their directions on their website cover installing them on my chair. First there is no "dust cap" to remove on top of the caster barrel to get to a "nylock" nut which they didn't send extras. There is however a "stembolt" that goes all the way through the caster barrel to a nut on the underside of the fork.

After removing the nut and sliding the forks off the "stembolt" mounted on the frog legs isn't long enough to go through the caster barrel. Plus there's no way to secure it if it did so what am I overlooking and not doing right?

I hope this makes sense, I'm desperate Pleas help!!

anyone tried allopurinol to prevent kidney stones? i have another giant kidney stone, jut had it removed in 2013 and its back. 2 inches long by 1 inch wide at one spot. i need a way to prevent these, i can see getting a stone but this size every 3 years. any other drugs that prevent kidney stones ?

Polycarbonate seat pan

The seat pan of my new chair was not well made, and it is too hard for my bottom even with cushion on.

So I cut away half of the welded aluminum seat pan, and replace with a piece of 3mm thick polycarbonate on top.

It works great, beside plywood, aluminum, polycarbonate is the most comfortable seat pan I have ever sat on. It will sink very nicely, almost contour with my bottom shape, it is not hard, but does not sack like cloth.

I don't know how long will this 3mm sheet last, hopefully as long as possible.

Freedom Ryder FRH 1A

There is a Freedom Ryder FRH 1A for sale in my area. I am wondering what I should look for when I check it out. It will be my first Handcycle. Its being offered for $1400 and I have no idea if this is a decent price or not. It looks like Freedom Ryder is no longer selling this model. Does anyone know if there were issues with it? Also, is this a more general riding bike vs a racing bike. I would eventually like to participate in local 5k races, more for supporting whatever is going on, not really racing. (At least not at this point :) ) Any information on this handcycle would be greatly appreciated.

I am a t6-t7 paraplegic, doesnt hav sensations and movement frm the waist. Yesterday nite i was affected by urinary tract infection. My sister s nurse aswellas my caretaker. She called for doctor, doctor asked to take urine culture report. In the meanwhile doctor asked to put 250mg amikacin sulphate. The lab technician says that v wud get the report only on wednesday. Today my stomach was little bit inflated, my sister chked me and said it s gas trouble. So she prescribed robo 20mg. The problem s the doctor went for some devotional trip. He wl availonly on wednesday. Also the fever was reduced, but once in 7 hrs am getting chilness, and at that time if i take one paracetamol 650mg, and it bcomes normal. The report wil come only after 3 days, now can i take robo20mg (rabeprazole sodium) while taking amikacin. Or my sister saying me to take 750 mg of amikacing ( now taking 500 mg of amikacin in two doses). Or shud i wait till the report. Nowomiting, no nausea.. waiting for ur reply.

Vocational rehab is pushing me towards independent living. I live in a little group home right now. I was living alone on my parents property in a little house but I had a lot of problems with caregivers not showing up on weekends so there were many times when I had to contact my family to help me out. My sisters have both grown up and moved out so there's really no one around all the time to help me out if need be. Does anyone live in an assisted living apartment complex? I'm looking to go in that direction to. I've got one other problem to top things off though, I have epilepsy so I have to watch out for seizures. The place I live now only comes to help me when I call for it so I'm not sure the big difference. I'm working with an independent living advocate to really plan things out and make sure it's possible. I live in Peoria right now which is roughly 25 miles And 2 cities away from anything I like to do and of course being epileptic I can't drive so getting places can take forever. I've been learning to use public transportation But it's restricted to cities and I would have to transfer. It's 100? outside so transferring is not really an option.

Avoid the possibility of having them fall in the 13? Gap shown below when using a standard transfer board on the right.
The safer transfer board is self supporting from the car and is removeable. After my wife embarasingly slid off the standard board twice, I designed and made the safer tarnsfer board (patent pending).
Get your plans to build your own board to custom fit your car. This could save the cost of buying a expensive wheelcair Van. The plans show exactly how to measure your car and how to cut each part for a good fit. If you have a Volt 2013 to 2015 you do not need to take measurements you can use the example measurements shown on the drawings.
Email tschraderpe.com if interested in having the plans. Or possibly having one custom made for you. I am looking for interest at this point.
Also have a leg stretcher, which has helped my MS wife with her cramped legs.

Hotel bed height

I recently took a road trip which had me staying at a number of hotels and Motels. In the past I have noticed that the greatest barrier to staying in regular rooms is inaccessible bathrooms - door too narrow or barriers around the toilet. (I am a L1 complete para) I have recently noticed that a number - especially of the higher end hotels have been raising the heights of their beds to the point where transfers are difficult and for many people impossible - I have even seen high beds in ADA rooms. Have others seen this?
